Become a Notary
Healthcare Rising Arizona is an organization that focuses on making real improvements to our healthcare system through ballot initiatives. Even when our politicians fail us, we have the opportunity to make our voice heard by directly passing policies together. Becoming a notary is an essential part of the citizen initiative process because Arizona requires that we validate all of the signatures we gather to get on the ballot in the first place.
The Steps
- Become a member of Healthcare Rising. For our team to process a notary application, you’ll need to become an annual or monthly contributor. Join your fellow Arizonans in improving our healthcare system today.

- Call our office at 602-354-8829. We will walk you through the next steps.
Note:
Filling out this form will only start the process of becoming a notary in the state of Arizona. You will need to coordinate with Healthcare Rising Arizona to make it official!
To become a notary, you must: be an Arizona resident, at least 18 years of age, able to read and write English, and a citizen or legal permanent resident of the United States.
You must NOT: have a conviction for a felony unless civil rights have been restored, or a conviction of a lesser offense involving moral turpitude or of a nature that is incompatible with the duties of a notary.
